According to the federal law "Health Insurance Portability and Accountability Act" (HIPAA), you have certain rights regarding the use of your individually identifiable health information. Only those members of your health care team with a legitimate "need to know" may access, use or disclose your information during your stay. Your signature authorizing OMC to release your information or request your personal health information from others may be required in certain circumstances. While receiving care at OMC, you may ask for your name to not be included in the hospital directory, which means that people asking for you will be told "I have no information about this individual." In order to receive deliveries of cards or flowers, your name must appear in the hospital directory. Additionally, if you include your name in the hospital directory, your name will appear on a list for clergy members of your faith.
